CHICAGO REGION ADVANCE CARE PLANNING COALITION

The Chicago Region Advance Care Planning Coalition was formed in 2005 to support the Someone to Trust advance care planning initiative.  The coalition includes local healthcare institutions (hospitals, hospices, healthcare systems), community groups (ethnic, cultural, spiritual and neighborhood), and policy bodies (Chicago Department of Public Health, Illinois State Medical Society, Metropolitan Chicago Healthcare Council, etc).

Participating organizations provide the expertise and volunteer time of their staff or representatives to the project.  Key coalition organizations are represented on the Advisory Council and Task Forces.  In addition, coalition organizations help disseminate information on the project to their audiences.
