Trained advance care planning facilitators can assist individuals and their agent in conversations that provide the information necessary to make medical decisions that reflect the individual's wishes. Facilitators can help individuals chose an appropriate agent, guide them through important discussions and assist in the completion of a legally valid, clear advance directive document.
The facilitators listed in this directory have successfully completed the Respecting Choices® facilitator training course and are located in the Chicago region. Individuals interested in having a facilitated advance care planning discussion(s) should contact the facilitator or organization directly.
