VISION

SOMEONE TO TRUST is a program of the Chicago Region Advance Care Planning Coalition.  The Coalition envisions a local culture in which all Chicago area residents routinely discuss their preferences for healthcare with their family, healthcare providers and other trusted individuals, and designate a power of attorney for healthcare.  The Coalition also envisions a healthcare system that can easily access and consistently honor residents’ healthcare plans. The Coalition aims to achieve these goals through implementation of the Someone to Trust comprehensive advance care planning program to train professionals, educate the community and prepare the healthcare system.
